Working as a Clinical pharmacist, you will be required to support with all aspects of medication management at both surgery and Primary care network level. You will be required to support practices with the interface between Hospital and Care homes teams. You will support other members of the PCN and practice teams by sharing pharmaceuticalknowledge. You will be required to support the proactive work aroundPrimary care network requirements including:- Provide Structured Medication Reviews (SMRs) to patients who are eligible for them Review patients who are prescribed medicines, alone or in combination, which have higher risk of harm such as dependency or gastrointestinal haemorrhage. Review patients who are prescribed DOACs, recording their creatinine levels, weight and calculating Creatinine Clearance to ensure the dose is correct Decrease avoidable prescribing of SABA inhalers for asthma patients by Increase use of inhaled corticosteroid (ICS) inhalers for appropriate in asthma patients to improve disease management and reduce unnecessary SABA use. Promote Environmental sustainability by Decreasing the use of MDI inhalers by prescribing dry powder inhalers (DPIs) and soft mist inhalers (SMIs) where clinically appropriate and agreed with patient through a shared decision making conversation Supporting cardiovascular disease preventions by support the Prescribing of statins to patients with higher CVD risk Identifying patients with high cholesterol levels and referring them for assessment for Familial hypercholesterolemia Reviewing patients with atrial fibrillation and ensuring that they are treated with DOACs in line with NICE guidance This is not an exhaustive list of the activites you will be involved in, please see attached job description
