Main duties of the job We are excited to offer an opportunity for a band 3 member of staff to join the Community Integrated Assessment Team, to work within the Integrated Intermediate Care team. The post is a permanent position and will be a part time post. The successful candidate will be expected to work as part of a multidisciplinary community based team, assisting with the delivery of therapy and rehabilitation to those patients who are referred to the Community Therapy Team. As this is a Trustwide post, the successful candidate may be required to work between the Stockton and Hartlepool localities. You will be expected to work flexibly and available for both extended day and seven day working.
