Part Time - 15hrs per week, Fixed Term - 6 months fixed - Mental Health Assessment and Review Service (MHARS)
We are looking to recruit to a Senior Practitioner role (Part Time, 2 days a week) based within the Mental Health Assessment and Review Service in Adult Social Care.
This is a great opportunity to work within a dynamic, person-centred, specialised team that engage with Service-Users in a fast-paced and therapeutic way.
It is an exciting time to be part of a specialist city-wide social work team working with individuals who experience mental health difficulties as well as those who are involved with the criminal justice system.
MHARS work alongside our colleagues in mental health to support the safe discharge of those with care and support needs from acute psychiatric beds. We also support the release of prisoners from HMP Bristol and the support planning of those returning to Bristol from out of area prisons.
You will support practitioners to:
* Specialise in Prison release work within the MHARS Team, but also be on hand to support the Leadership Team across the Team.
* Work with adults with mental health difficulties, substance misuse, homelessness and other associated difficulties.
* Work in a fast-paced way due to the nature of our referrals, responding to needs in relation to discharges and releases from acute hospital beds and prisons respectively.
* Undertake Care Act assessments and planned and unplanned reviews as appropriate.
* Undertake s42 (Care Act) safeguarding enquiries.
* Maximise use of social capital, reablement support and technology enabled care to undertake support conversations, reassessment where required and update support plans.
* Support & advise practitioners in weekly ward rounds/bed management meetings; CPA meetings / Blue Light Meetings and any other family and/or multi-disciplinary meetings including MARAC/MAPPA.
You will have support from the other Senior Practitioner and Team Manager & Service Manager.
You must have an interest in and experience of working in a multi-disciplinary way, be able to work quickly and proportionately and have confidence in respect of decision making and positive risk taking.
The Senior Practitioner role involves supervising social care practitioners, social workers, and overseeing the development of ASYEs & student and apprentice social workers; being part of the leadership team and undertaking strategic work; and building relationships with internal and external partners. You will need to be a good communicator, with excellent time management skills.
A relevant qualification in social work or occupational therapy is essential.
