Job Overview
We are seeking a dedicated and compassionate Care Coordinator - Nights to join our team. The Care Coordinator will play a vital role in ensuring that patients receive the highest quality of care by facilitating communication between patients, families, and healthcare providers. This position requires strong organisational skills and the ability to manage multiple tasks efficiently within an office environment.
Duties
1. Serve as the primary point of contact for patients and their families, addressing inquiries and concerns regarding care plans.
2. Coordinate patient appointments, ensuring timely access to necessary services and resources.
3. Maintain accurate and up-to-date patient records, ensuring compliance with all relevant regulations and policies.
4. Collaborate with healthcare professionals to develop and implement individualised care plans tailored to each patient's needs.
5. Monitor patient progress and follow up on care plans, making adjustments as necessary in collaboration with the healthcare team.
6. Provide education and support to patients and families regarding treatment options, resources, and community services available.
7. Assist in the development of policies and procedures that enhance the quality of care provided.
Experience
1. Previous experience in a healthcare or office setting is preferred.
2. Strong communication skills, both verbal and written, with the ability to convey complex information clearly.
3. Have or working towards NVQ Level 3.
4. Excellent organisational skills with a keen attention to detail.
5. Proficient in using office software applications and electronic health record systems.
6. Ability to work collaboratively within a team while also being self-motivated.
7. A compassionate approach towards patient care, demonstrating empathy and understanding at all times.
