The team are based in Ipswich and you will be working with one of the most complex demographics within Suffolk. You'll be working alongside a multi-disciplinary team, undertake the role of the duty worker as part of our team's rota. You'll regularly be working with external agencies as part of a service users care, so being able to work well in a team and having excellent communication skills is vital. Main duties of the job You can expect a manageable case load and a supportive management team to ensure you're able to work independently but support is never far away if needed. Working closely with the ACS mental health team to support our patients, meeting both their mental health needs and their Care Act needs. To support patients in being discharged back to primary care.
