This is a fantastic opportunity to become a part of a well-established community multi-disciplinary team, covering a diverse range of service users with multi-morbidities including, but not limited to: Stroke, progressive neurological conditions, orthopaedics, falls, frailty, and complex multi-pathologies.
Main Duties of the Job
* Manage autonomously a clinical workload of patients/clients at a specialist level, assessing and treating patients.
* Determine clinical diagnoses and develop, deliver, implement, evaluate, and modify individualized treatment programs.
* Specialist assessment and treatment skills in areas including Stroke, Progressive Neurological Disorders, Orthopaedics, Falls, Frailty, and Self-management.
* Set SMART goals to ensure effective treatment and improve patient flow through the service.
* Work collaboratively within the MDT and participate in the supervision process with staff and students.
* Participate in governance initiatives, measuring and evaluating clinical practice.
* Ensure the safe use of equipment and adhere to service policies.
Person Specification
Qualifications / Training
* Degree/Diploma/MSc in Physiotherapy
* HCPC Registered
* Trained Clinical Educator
* Experience of developing and delivering training sessions
* Masters level qualifications in a specialist area
Knowledge, Experience & Expertise
* Recent experience in rehabilitation and long-term condition management
* Experience supervising and training others
* Knowledge of relevant legislation and best-practice for a range of conditions
Communication and Relationships
* Ability to communicate effectively with people with complex difficulties
* Self-motivated with the ability to work independently
* Excellent time management skills
