An exciting opportunity has arisen for an individual to join our team as a Telecare Assessor on a one-year fixed term contract. The successful candidate will develop an enhanced skill set that is transferable throughout the NHS and other public sector roles within a Health and Social Care setting. Full training and regular supervision will be provided. This position is being offered on a fixed term / temporary basis, but there may be scope for this role to become permanent.

The role requires travel throughout the County, visiting vulnerable adults with varying disabilities and needs. Therefore, access to a vehicle will be essential for this role.
Main duties of the job
The successful candidate will be responsible for assessing individual needs by visiting individuals in their own homes, hospital, or other care environments and will be expected to effectively manage their own diverse caseload. The post holder will prescribe Telecare Technology solutions and look at situations holistically and refer onto other services, where needed. This will offer a great insight into the tech world that we are entering and see how technical solutions can help support those with Health and Social Care needs.
The Specialist Telecare Assessor is responsible for the assessment, review and re-assessment of assistive technology packages, including basic installation of Telecare devices / sensors.
The post holder will use their practical and technical skills to problem solve, evaluate and alter Telecare care plans within set eligibility criteria. They will also install and fault find on basic Telecare equipment over the telephone and within a person's home.
The ideal candidate will be a team player, will be prepared to work hard, display a positive & caring attitude, understand a range of health conditions, and be able to effectively communicate with both service users, their family and other health & social care professionals.
Job responsibilities
The post holder will plan, assess, deliver and evaluate intervention both over the telephone and face to face in a community, Hospital or care setting.
The post holder will work using Community Care legislation and guidance as well as within the guidance of the Telecare Service Governing body (TSA) and provide appropriate assistive technology interventions within the persons home.
Person Specification
Qualifications
* NVQ Level 3 or equivalent health or social care qualification or equivalent experience within a health or social care role
Experience
* Significant experience working in a Heath, Social Care, or equivalent personal experience of caring for others
* Experience working with vulnerable adults
* History of carrying out an assessing role
* History of managing own caseload
* Experience of using Liquidlogic system
Knowledge
* Willingness to learn about assistive technologies, increasing knowledge and developing within the role
* Ability to problem solve, offering creative solutions to unique situations
* Experience of working with assistive technology
* Experience of equipment prescription, working autonomously to reach and commit to appropriate equipment solutions

* Access to a car and travel countywide
