Job Overview We have an exciting and rare opportunity to join the Southeast Staffordshire Rehabilitation Team based at Samuel Johnson Hospital, Lichfield. The team consists of Physiotherapists, Occupational Therapists and Specialist Nurses providing intervention to adults within the Burntwood, Lichfield and Tamworth locality. Referrals to the service cover a range of conditions including traumatic brain injury, Stroke, Multiple Sclerosis, Parkinson’s, MND, other neurological conditions, spinal cord injury, falls, orthopaedic conditions, amputations, hip and knee replacements, arthritis, fractures, early onset dementia and multiple pathologies. Main duties of the job You Will Be Required To Work on your own as a registered practitioner within the established multidisciplinary team, you will undertake a Community Physiotherapist role for the South East Staffordshire Community Rehabilitation Service. You will be expected to assess, identify, and agree goals and treatment plans using diagnosis and evidence-based treatment programmes to meet a wide variety of patient needs in the most appropriate location in the community e.g. patients’ own homes, clinics, and care homes. The role will involve supporting the team to provide training to carers, care agencies and other external partners on an ad hoc basis at the request of Operational and/or Clinical Leads. The post holder will hold responsibility for their own caseload and will be required to work without direct supervision. They will undertake regular supervision and training of rehabilitation assistants and supervise, educate, and assess the performance of Physiotherapy students. Using clinical reasoning skills you will provide accurate clinical diagnosis, agree physiotherapeutic goals and the physiotherapy treatment plan with the patient/carer. To work to CSP and HCPC code of professional conduct and to work within guidelines. To have a good working knowledge of national and local standards and monitor own and others quality of practice. To be responsible for maintaining own competency through Continuing Professional Development activities and maintaining a portfolio which reflects personal development and meets the HCPC standards. To maintain and develop current knowledge of evidence-based practice, developing specialist knowledge of conditions and patient types through in-service training, self-directed learning, tutorials, peer review and external courses. To hold responsibility for own specialist caseload and be responsible for a defined area of service / patient type working without direct supervision as an autonomous practitioner. To act as a source of spontaneous and planned clinical expertise, giving appropriate advice and guidance for the patient, carers and other professionals within health, education, social services, and the voluntary and private sectors, within safe practice guidance. Provide and receive complex information relating to patients’ diagnosis, treatment and long-term rehabilitation outcomes to patients, their carer’s and the multidisciplinary team. To use communication tools (verbal and nonverbal) to gain informed consent with patients where there may be difficulty with effective communication e.g. limited comprehension altered perception, pain, fear, and/or those who may be unable to accept diagnosis. To use highly developed communication skills to give both spontaneous and planned advice to disseminate sensitive and emotive information in such a way that the patient understands their diagnosis and expectations for independent function and to agree goals and gain co-operation in the ongoing treatment plan. Person specification Qualifications & Training Essential criteria BSc in Physiotherapy. Professional registration (HCPC).
