Social Worker - Pathways Team
We are looking for a Pathways Social Worker to support young people and adults as they move from children's services into adult care. This is a crucial role in ensuring individuals experience a smooth and well-supported transition, enabling them to achieve independence, stability, and the best possible outcomes.
Key Responsibilities:
* Conduct comprehensive assessments and create tailored transition plans.
* Provide guidance and advocacy to young people and their families throughout the process.
* Collaborate with multidisciplinary teams, including education, health, and social care professionals.
* Ensure compliance with safeguarding policies and care legislation.
* Maintain accurate records and provide detailed reports to inform decision-making.
* Promote independence, inclusion, and person-centered care approaches.
About You:
* Hold a social work degree.
* Right to work in the UK and hold a clean UK driving license.
* Registered with SWE.
* Post-qualified experience within children and family services.
