Join the Knowsley High Intensity User Service team as a Support Worker and make a difference to individuals experiencing mental or physical health crises.
As part of our team, you'll liaise with clinical teams to discuss referrals, manage risk, and implement structured support and safety planning programs. You'll maintain accurate referral databases, support individuals in building life skills and resilience, and link them to community networks and peer support opportunities. We provide crucial out-of-hours support helping to prevent the need for A&E visits through early intervention and adequate support.
Working in partnership with various agencies and community groups, you'll help improve health and wellbeing for our service users, ensuring they receive inclusive and non-discriminatory care that meets their diverse needs.
