Job summary
As a Practice Nurse you will be responsible for the provision of safeand efficient primary care appointments in line with PrimaryMedical Services GP Contact, with full access to patient medical records. Theywill provide a caring service to the practice patients toinclude treatment, preventative care, screening and patient education. Theywill also support the delivery of QOF, CQUINs, locally commissioned services,coding consultations and seeking opportunities to ensure the delivery of theseservices. The post holder will take a pro-active approach to healthcarebe accountable for their own CPD, contribute to continuallyimproving the clinical effectiveness of the clinics and ensure teamwork by respecting andworking collaboratively with all colleagues.
5 hours per week
Location - Segensworth (travel to sites across Hampshire and Isle of Wight to conduct Face-to-Face clinics)

Provide TreatmentRoom services and others, dependent upon Competency level. To include, but not limited to: wound care, cervical cytology, swabs,family planning such as contraceptive initiation and checks, Chronic DiseaseManagement (Asthma, COPD, Diabetes)
Screening. Undertake opportunistic screening including BP, height, weight, BMI,urinalysis, smoking status and alcohol intake.
Health Promotion. Offer education and motivation for patients to change unhealthybehaviour. Recognise a patients readiness to change. Develop and maintainhealth promotion material.
Infection Control. Ensure that the highest infection control standards are met.
EmergencyManagement. Be competent in anaphylaxis andresuscitation techniques
Be aware of recognising the need forsafeguarding children and adults, raising concerns appropriately.
Administration andpaperwork. Arising directly from this caseload(record-keeping, other written communications with local GP surgeries asrequired) and ensuring all appropriate templates and coding is completed forQOF, LCS, DES
Team meetings. Essential to the delivery of team based care, contributing todiscussion of clinical practice standards, developing protocols, mutualprofessional support for colleagues, will need to attend meetings and activelyparticipate in these.

The post holder will be expected tocontribute to and support the development of the service.
It is expected that the post holderwill:
Communicate with the Governance Lead andsupport the delivery of a high-quality service and raise any governance issuesthat may arise.
Work with the Service Manager to ensuretraining, significant events, complaints, audits, surveys and otherprofessional service development activities are achieved to ensure the smoothrunning of the service.

Experience
Essential
1. Registered with NMC / HCPC
2. Experience working in a primary care
3. Due to the nature of the role, mental health is desirable
4. At least three years post qualified
5. At or working towards degree qualification / or equivalent experience
6. You must be competent at providing treatment room tasks such as immunisations, wound care, chronic disease management
7. You will be expected to ensure that mandatory education is kept updated as required by the policies of the organisation
8. Excellent communication and interpersonal skills written, verbal (Particularly listening skills)
9. Clear understanding of professional responsibility and accountability.
10. Good organisational skills and ability to manage and prioritise own workload
11. Good clinical skills and documentation skills using clinical IT systems
12. Be able to clear an enhanced DBS check
