Main duties of the job
* To provide clinical assessment through history taking and a mix of preventative (proactive) and acute (reactive) care planning.
* To undertake holistic assessment of the patient's individual needs and refer within the wider team or community.
* To contribute to peer learning and education via significant event analysis and case-based discussions at MDT meetings weekly.
* To provide support, clinical advice, supervision, and feedback to students and other members of the MDT.
* Record data and assessments in patient records systems promptly and accurately and to agreed standards ensuring appropriate use of read codes and templates, with awareness of QOF targets and local DES/LES specifications.
* To compile and issue computer-generated acute and repeat prescriptions, prescribing in accordance with BNSSG prescribing formulary whenever this is clinically appropriate, working with our pharmacy hub or to work towards a prescribing qualification.
* To contribute and bring ideas for continuous improvement including developing/improving care pathways and contributing to QIP and audit.
* Participate in weekly MDT meetings dementia/mental health, palliative care, or complex care with support of the team and our community mental health. Bring case discussions.
* To provide outreach in the form of acute home visit assessments.

Working each day from askmyGP and assigned caseload.
Meeting at 11 am to discuss any difficulties experienced with patients or diagnostic doubts.
This work takes place via a variety of formats: telephone, video, email, and face to face.
Career and personal development promoted through regular weekly meetings with a GP mentor/appropriate university training/peer-to-peer learning.

Person Specification
Experience
Essential
* At least 5 years experience of working within a community or emergency healthcare setting at Band 6 AfC equivalence or above.
* Substantial experience with complexity.
* Autonomous working (within scope of 4 pillars of advanced practice).
* Experience teaching students, team members, and peers.
* Experience of multidisciplinary working.
* Knowledge of current clinical evidence-based practice.
Desirable
* Primary Care, Community nursing and therapies, or emergency care practitioner background.
* Caseloads and MDTs.
* Audit and continuous improvement experience.

Qualifications

* Registration with health Governing Body (eg NMC, HCPC).

* Advanced Practice MSc.
