Key Responsibilities
• Direct Patient Care:Administer medications and monitor vital signs for patients with various medical conditions.
• Assistance with Daily Activities:Support patients with personal care and daily living tasks to enhance their comfort and independence.
• Collaborative Care Planning:Work with a multidisciplinary team to assess patient needs and develop effective care plans.
• Documentation:Maintain accurate patient records in line with NHS guidelines.
• Patient and Family Education:Educate patients and families about health conditions, treatment options, and discharge plans.
