We are seeking a qualified Social Worker to join the Short-Term Assessment and Reablement Team, working across South Derbyshire.
Applicants should have a Social Work qualification and experience in an Adult Social Work team setting. Demonstrating a person-centred and strength-based approach to help individuals live as independently as possible within their homes and communities is essential. Experience in a collaborative, multidisciplinary environment to support individuals in returning home or remaining at home following health or needs changes is required.
The candidate must possess knowledge of current legislation, policies, and procedures underpinning good practice, along with an understanding of Safeguarding principles.
