A fantastic opportunity has arisen for a Specialist Clinical Practitioner - Nursing within the Tiverton Core Adult Mental Health Team based at Tiverton Community Hospital.
You'll be joining Devon Partnership Trust, which encompasses urban, rural and coastal demographics that combine to make this a fascinating and beautiful area to work.
The successful candidate will be joining a dynamic, motivated and skilled service and help the team in the Tiverton core team to build upon the successful work they have and continue to achieve. You will also be part of the exciting work we are undertaking as we implement Care Pathways associated with psychologically informed interventions for all clients referred to the community team.
The Tiverton core team are proactively now engaging with the local Primary Care Network, working alongside the local GP Surgeries.

Main duties of the job
Youwill be based within the Multi Agency Team supporting the needs of the PCN -increasing the availability of appropriate interventions to individualsexperiencing an episode of mental disorder.
Thepost holder will facilitate an increase in the proportion of patients referredto IAPT services & other primary care focused mental health services (suchas those offered via the VCSE alliance) with a reduction in the proportion ofpatients stepping up to provision for those with more complex needs.
Youwill assess individuals without referral from primary care colleagues offeringassessment of complex individuals, outlining management plans, deliveringinterventions independently where necessary, & coordinating broaderpsychosocial interventions.
Theywill also support the assessment practitioner where required.
Thepractitioner will have a time-limited capped caseload of individuals receivingdirect care which they will be responsible for managing. The individualwill also support patients being transferred from core teams after a successfulperiod of treatment or stabilisation.
TheSpecialist Clinical Practitioner (SCP) will work within a number of GP practices inthe PCN & in collaboration with the MHConsultants, Clinical Directors & GPs determine the most effectiveallocation of time to each GP practice. Over time the expectation is of adeveloping mental health response across the PCN & the SCP will play a central role in identifying unmet need in thepopulation

Person Specification
Qualifications
* Clinical based Degree. Qualified as RMN.
* Minimum of 60 credits at post-graduate level in relevant subject ( e.g. Advanced psychosocial assessment and formulation, Enhancing the Care and Support of the Person with Dementia and their Carer)
* Post basic qualification in speciality Application Non-Medical Prescriber (V300)
* Evidence of Continuous Professional Development
* Current registration with the NMC and holds current registration with no restrictions.
Knowledge & Skills
* Understanding of Community Mental Health Service Framework transformation
* Awareness of challenges, legal frameworks in delivering mental health within primary care
* High level communication skills including: Presentation and facilitation skills; Consultation and engagement skills.
* An ability to adjust communication effectively to meet the needs of a diverse group including internal and external stakeholders.
* Ability to travel across Devon and other locations in the course of the business requirement.
