Eden Brown Synergy has an exciting opportunity for a Social Worker to join a multidisciplinary team offering mental health and rehabilitation services to male patients in low to medium secure and acute units.
The Social Worker would have previous NHS or private care experience and would be required to manage a complex caseload, responsible for social care assessments, risk assessments, facilitating group activities, and will contribute to ICR meetings, supporting service users to make viable plans for their future. The Social Worker will be instrumental in encouraging individuals to lead a full and valued life, enabling them to live more independently.
The Social Worker would report to the Safeguarding lead and manage internal investigations while liaising with home authorities for service users during their admissions and discharging process.

Key Responsibilities:
1. Educating Hospital staff about relevant safeguarding matters.
2. Completing a full annual social care reassessment within the CPA format.
3. Facilitating access to welfare rights, housing, advocacy services, and associated support services.
4. Participating in 1:1 supervision.
To be considered for this role, the Social Worker would need:
1. A Registration with Social Work England.
2. A Social Work degree.
3. Experience in a similar environment is preferred.
