The Homeless Nurse will work within a multi-disciplinary team to deliver high quality care and support the individual’s experiencing homelessness. The role involves providing health assessments, support with managing complex health conditions and liaising with other services to ensure a holistic approach to care both in the primary care setting or the hotels they reside in.
Clinical care:
Conduct health assessments and provide a person- centered care for the individual who are homeless or at risk of homelessness.
Administer treatments, medication, immunisations as required, including wound care, infection control, management of long term medical conditions and support around mental health, substance misuse and other complex health issues.
Secondary responsibilities:
Liaise with outside agencies to support the patient with their health and social needs.
Act as an advocate for their individual health needs
Participate in supporting the practice with GP and Nurse trainees as Brownlow is a training practice
Attend meetings and conferences relevant to the role
Participate in monthly educational meetings
Undertake mandatory training and educational training to ensure delivering safe, effective and up to date care.
