The team provides psychologically-informed housing and resettlement support, tailored to address the specific needs of individuals. Our forensic mental health step-down services are spread across four sites, offering both high and medium support for men and women. In this role, you will play a vital part in supporting the successful rehabilitation and reintegration of our residents into the community. You will deliver pathways, support, and targeted interventions to facilitate successful resettlements, contributing to safer, more integrated communities. What we are looking for:
9. Experience of working with people of complex backgrounds
10. IT proficiency, ability to learn new software programs, and basic Microsoft experience
11. Understanding of the housing and social needs of people with multiple and complex needs
12. Ability to influence and negotiate positive outcomes
What we would like, but not essential:
13. Appropriate professional qualification: NVQ/Diploma Level 2 in Health & Social Care/Community Justice or professional equivalent
14. Experience providing housing support and practical assistance in residential or outreach support roles
15. Experience creating co-produced support plans, providing appropriate interventions, and liaising with other professionals
KEY RESPONSIBILITIES
16. Act as a Recovery Worker for a caseload of Residents and Participants (R&Ps), contributing to the development of support plans, risk assessments, and reviews.
17. Provide advice, guidance, information, and life skills training to R&Ps.
18. Conduct welfare checks to ensure smooth service operation.
19. Ensure R&Ps understand their rights and responsibilities and have access to the necessary tools and resources for community reintegration, including support with welfare benefit claims and signposting to appropriate services.
20. Develop and maintain strong relationships with internal and external stakeholders and agencies.
21. Support R&Ps with medication management.
22. Assist with health and safety and other site-related maintenance and management.
23. Perform various administrative duties as required.
