Job responsibilities · A truly holistic approach by consulting with the patients GP and other involved healthcare providers to develop and implement a medication plan that can meet the overall goals required for the individual patient. · Running patient facing clinics using their clinical experience to solve health problems through the rational use of medications and telephone consultations for prescribing queries · Home visits to perform medication reviews on patients who are unable to come to the surgery. · Patient focused health education to improve or maintain health such as exercise, diet, immunisation, smoking cessation. · Medication reviews for patients on multiple medications which include Quality and Outcomes Framework reviews for patients with long term conditions · Post discharge medication reconciliation and timely follow up reviews to ensure improvement in outcomes for all patients who have been in hospital. · Care home prescription management programme, requiring an active attendance within the care home setting to provide face to face care of on-going health issues in an elderly and vulnerable population sector · Involvement with MDT in palliative care · Chronic disease management by monitoring of BT results focusing on potential harm that can come from long term use of medicines or poor compliance. · Medicines optimisation, helping the practice to achieve its annual medicines optimisation incentive payments, including improving prescribing of antibiotics and blood thinning agents. · Management and regular review of all patients on shared care prescribing of DMARDS from secondary care. · Attendance at clinical meetings with GP’s to facilitate dissemination of information. In addition to the primary responsibilities, the clinical pharmacist may be requested to: • Partake in audit as directed by the audit lead • Provide education on medicines for the clinical team • Review the latest guidance ensuring the practice conforms to NICE, CQC etc. • Act as the practice point of contact for all medicines related matters, establishing positive working relationships This role will be given the opportunity to provide a level of project management working as part of a team and on an individual basis. It is anticipated that the post holder will be responsible for planning, reporting and delivery on projects.
