Job overview
The objectives for the team are to improve the patient experience, enabling efficient flow of patients through the pathway and therefore reducing length of stay. The team will ensure all discharges are planned as soon as possible in a patient’s acute stay and to guarantee that when patients are ready to be discharged, that the most appropriate services are available to pull patients back into the community.
Main duties of the job
* The post holder will facilitate an effective discharge practice by providing support and expert nursing advice to all members of the discharge team, patients and their carers. In collaboration with colleagues across the Transfer of care Team, SPA, Frailty Unit and Community Services, provide specialist information about the services to help facilitate effective and safe patient pathways. The aim is to provide a unified, effective service.
* Provide support to the Operational Team Lead and the Senior Transfer of Care Sister / Charge Nurse during the initial set up of the new integrated discharge service and ongoing support thereafter.
* To deputise for the Senior Transfer of Care Sister / Charge Nurse in their absence as appropriate.
* To support the Transfer of Care Team implement any operational change in the service.
* To support the Senior Transfer of Care Sister / Charge Nurse in prioritising workload and allocating team members and resources.

* Current NMC Registration
* ENB 998
* Evidence of further study to post graduate level or the equivalent level of expertise in the speciality
* Evidence of commitment to professional development and training
* Comprehensive knowledge of legislation relating to hospital discharge

* Teaching & assessing qualifications
Experience
* Significant experience in an appropriate specialty (community)
* Experience of collaborative working within a multi-disciplinary team
