Hours: 37.5 hours, 5 days over a 7-day period, with varying shift patterns as agreed with the Manager
About Us
Sandant Care provides personal care and support within two specific service areas in the Solihull area. We provide care and support for individuals in their own homes and supported living, plus support in the community. Along with personal care, we provide housing-related support services – working alongside the person to manage their home, including guidance and support paying bills, securing the home, and managing the tenancy. We strive to deliver the highest quality care standards and outstanding service. We provide care packages for vulnerable people living within the community, supporting those with learning disabilities, autism, mental health issues, and older people.
The Role
The role of the Enablement Support Worker is to work in the community, supporting service users and helping them become as independent as they can. Helping and supporting Service Users with different physical disabilities and mental health needs to live their lives more independently and support them to reach their potential by providing both physical and emotional support. The role of an Enablement Support Worker is varied - each person has unique needs, which makes the job unique too.
Main Function of the Job
* To ensure that recovery goals identified using Planning.
* To encourage service users to recognise, understand and manage factors that affect their mental wellbeing and physical health.
* To increase choice and involvement of service users through access to peer support and volunteering opportunities.
* To empower individuals to identify and overcome obstacles to success in achieving their self-determined goals, engendering self-belief.
* To record and feedback to clients about their progress and achievements and to identify further goals which help them achieve their aspirations.
* To work with a range of partners and external agencies to ensure clients benefit from personalised and joined-up support.
* To work as a team member, sharing information and supporting colleagues and volunteers, ensuring they have the knowledge, information, and resources to be successful in their roles and make a positive and valued contribution to the service.
* Participate in the monitoring requirements of the service by providing statistical and qualitative information on activities, success stories, and outcomes achieved.
* To maintain accurate records as required by policies and procedures, using appropriate paper and electronic systems.
* To maintain timely, accurate details of clients' service and support activities and outcomes achieved.
* To engage with community events, meetings, forums, and local organisations and networks.
* To proactively contribute to continuously improving the service by making positive suggestions, providing constructive feedback, and assisting in the implementation of agreed new ways of working.

* Maintain up-to-date knowledge of legislation, national and local policies and procedures in relation to Mental Health and Primary Care Services.
* Actively promote mental health issues positively.
* Ensure good communications both within Sandant Care and external agencies.
* All employees have a responsibility and a legal obligation to ensure that information processed for both patients and staff is kept accurate, confidential, secure, and in line with the Data Protection Act (1998) and Security and Confidentiality Policies.
* Maintain the confidentiality of all electronically stored personal data in line with the provision of the Data Protection Act.

To be successful in this role, you must have the following skills/experience:
* NVQ Level 2 in Health & Social Care.
* Experience of working in a social care setting or able to demonstrate a lived experience.
* Experience of identifying and co-ordinating a range of services to meet individual needs.
* Up-to-date knowledge of sector-specific legislation and best practice.
* Ability to maintain electronic records.
* Clear verbal and written English and excellent communication skills.
