· To work in conjunction with Adult Community Health and Specialist Palliative Care nurse teams to provide hands on care in the community setting to patients who are nearing the end of their lives.
· To providepersonal and social care forpatientswith life-limiting illnesses
· To work with community care to support people to die in the place of their choosing, avoiding unnecessary admission of patients at the end of life.
· To provide psychological support for patients and their families/carers.
· To work as an effective team member collaborating with the other health and social care teams.
· To assist in the timely discharge of patients from the hospital or hospice, this may be because the patient suddenly wants to die at home or because continuing care or social care cannot immediately be implemented.
· Participate in weekend rotas 7 day working rotas and night rotas.
· Maintain high standards of care and expertise
· Provide practical caring skills and advice to patients who need physical support during their illness e.g. personal care, psychological support etc.
· Provide basic food prep.
· After appropriate training, provide more specialised care e.g. stoma care, simple lymphatic massage, phlebotomy etc.
· Be aware of various clinical interventions and common drugs and assist patients in taking medication if appropriate.
· Work within the care plan and accurately record actions in nursing notes.
· Work within agreed guidelines and protocols.
· Escalate any safeguarding concerns to the Hospice @ home team leader/ Matron and the identified Trust lead ensuring that appropriate guidelines, recommendations are adhered to and communicated to the hospice @home team.
· Be able to communicate effectively at all levels with patients, families and professionals.
· Provide psychological support and empathy to patients, their relatives and carers, during their illness, at the end of life. This will often be in emotionally charged circumstances including time of death
· Assist in establishing close liaison and communication with, and between, all disciplines involved in the care of the patients and those close to them.
