The Community Nursing Teams in Brent are seeking Band 7 District Nurse Team Leaders responsible for providing clinical leadership in the delivery of high-quality care for the patients in the community setting and the day to day management of individuals within a team.
Main duties of the job
Manage care of patients referred by the group of General Practitioners (GPs) aligned to the service, with the aim of preventing unnecessary hospital admissions, in line with the commissioner's objectives. This includes on-going support and expertise through comprehensive assessments, individualised care plans, delivery, evaluation, and coordination of care between other agencies involved in the patients' care.
Communicate with GPs, participating in regular meetings and keeping GPs informed of patients' development. Work closely with GPs to plan care and advanced treatment that reduces the risk of inappropriate hospital admissions.
Demonstrate and maintain clinical credibility through the provision of high standards on clinical care using current evidence-based practice. Apply clinical leadership to continuously review and change practice demonstrating good and efficient clinical care.
Work with the Clinical Development lead to organise and develop training programmes for District Nurses.

Education/Qualifications
* 3-year District Nursing experience at band 6 level.
* District Nurse/Teaching qualification.
* Nurse prescribing appropriate to role.
* Evidence of continued professional development, with courses relevant to area of work.
* Evidence of team management.
* Management qualification/or willingness to work towards.
* Evidence of working towards a degree.
Knowledge & Experience
* Experienced mentor at pre and post registration level.
* Experience of nurse leadership and management.
* Experience of appraisal and clinical supervision.
* Experience of managing a team.
* Experience of dealing with difficult situations.
* Experience in PDP and setting objectives.
* Sound knowledge of current issues and legislation within the NHS.
* Previous experience of team management and multidisciplinary working.
* Participation in research and clinical audit.
* Clinical Governance and risk management agenda.
* Understanding of budget and performance.
* Experience of developing strategies, clinical guidelines, and policies.
