Job overview
The role is to support Band 6 nurses in the Dudley Special schools and the Community Children’s nursing team. The nurse will support in managing the health needs of children and young people with complex health and profound learning disabilities.
Main duties of the job
The nurse will support in managing the health needs of children and young people with complex health and profound learning disabilities.
* Working closely with the MDT to ensure that Education, Health and Care Plans are implemented.
* Conducting home visits and other community visits as part of holistic case management.
* Providing clinical skills and ensuring a holistic, family-centered approach to assessing, planning, implementing, and evaluating the health needs of the children.
* Teaching/training and supporting the team as well as the school staff to ensure the health needs of the children are met.
* Implementing strategies to support the care of children with learning disabilities.
* Managing emergency health situations in the school.

Detailed job description and main responsibilities
Main Duties/Responsibilities
* Demonstrate empathetic communication skills when dealing with patients and carers.
* Provide specialist nursing advice and support to families of children with significant medical needs under the direction of the band 6.
* Complete home visits as required to assess the child’s clinical condition.
* Undertake specific clinical tasks and skills for which the nurse has received training and is deemed competent.
* Encourage children and families to participate in their care by providing training and support.
* Implement agreed transition of care processes ensuring it takes place in a timely fashion.
* Have knowledge of the SEND agenda and contribute to EHCP’s and the reviews.
* Clinically assess the child or young person and ensure that actions are taken to promote safety.
* Support the safe administration of medications within the school.
* Provide and maintain a high standard of skilled nursing care for children with additional health needs.
* Complete accurate and timely recordings of involvement with the child and family.
