About the Role Rayleigh & District PCN is looking to recruit an advanced clinical practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries. This role will work as part of this large team that includes advanced nurse practitioners, advanced clinical practitioners, paramedics, care coordinators, social prescribers, pharmacists, pharmacy technicians, nurse practitioners and healthcare assistants. You will be an experienced general practice pharmacist and qualified advanced practitioner, who acts within their professional boundaries, supporting and working alongside the primary care network (PCN) clinical pharmacy team to develop, manage and mentor them. You will work as part of a multi-disciplinary team and provide clinical pharmacy and advanced practitioner appointment slots for the network, proactively and competently managing acute and long-term conditions, prescribing/referring where appropriate to do so. The post holder will:
1. Work towards health promotion and prevention and comprehensively assess patients for risk factors and early signs of illness.
2. Draw on diverse range of knowledge in your decision-making to determine evidenced-based therapeutic interventions, which will include prescribing medication where legally allowed and actively monitoring the effectiveness of therapeutic interventions.
3. Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, and provide direct support to patients and clients.

About the Candidate

Qualifications, skills and experience required:
4. Active Health and Care Professionals Council (HCPC) Registration and license to practice.
5. Master’s degree in pharmacy (MPharm) (or equivalent).
6. MSc in Advanced Practice or equivalent.
7. Non-Medical Prescribing.
8. Advanced Skills in managing long term conditions.
9. Previous experience working as a senior clinical pharmacist with a minimum of 4 years’ experience (or equivalent) as a pharmacist, with at least 2 years in general practice.
10. Evidence of recent teaching, mentorship, and supervision of staff.
11. Advanced skills in listening and information processing, alongside empathetic skills to ascertain, understand and respond to individuals’ complex needs and concerns.
