Job summary To clinically assess patients for NHS Continuing Healthcare eligibility in patients' homes, care homes, hospitals. To follow process laid out in National Framework for NHS Continuing Healthcare. Participate in MDT eligibility recommendations. Ensure assessments are robust. Complete new referrals for NHS Continuing Healthcare within 28 days as per the National Framework for Continuing Healthcare. Main duties of the job Carry out joint assessments with Social Service colleagues to identify individuals who care needs meet the criteria for NHS Continuing Healthcare. Proactively manage a delegated caseload and have the ability to function autonomously. Demonstrate timely forward planning and an ability to organise self. To complete relevant specific core reviews/assessments for NHS funded individuals e.g. Deprivation of Liberty/Liberty Protection Safeguards. Deal with people with complex needs and challenging behaviour, and people approaching end stage of life. Deal with difficult family situations and dynamics which require a high level of competent effort, and the ability to remain objective and use reflective practice and team support.
