We are seeking a highly motivated and compassionate Community Psychiatric Nurse to join our multidisciplinary team.
You will play a crucial role in providing holistic care to individuals with diverse mental health needs within the community.
Key Responsibilities:
* Conduct thorough holistic assessments of individuals' mental and physical health needs, social circumstances, and support systems.
* Develop, implement, and evaluate personalized care plans in collaboration with service users and the multidisciplinary team.
* Coordinate and deliver a range of therapeutic interventions, medication management, and practical support.
* Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
Main duties of the job
* Arranging, contributing to and documenting care reviews, including telephone, home visits, video and face to face reviews – as appropriate and defined within the care group SOP.
* To supervise junior staff members and to support students in placement.
* To work collaboratively with the multidisciplinary team as well as patients, carers, family members and internal and external agencies.
* Effective communication skills and self-governance are required. You will be able to demonstrate knowledge of high-level interventions & community assessment skills. Existing qualifications or a willingness to undertake training relevant to the post is expected.

* Recording and reporting accurately any changes related to the patient’s current behaviour and/or risks to the Nurse/healthcare professional in Charge in a timely manner.
* Recording care given and reporting omissions accurately and in a timely manner.
* Supporting and carrying out therapeutic observations as required and instructed.
* To make recommendations to the care plan process.
* Promoting a healthy, safe and secure environment by adhering to Trust statutory health and safety requirements and maintaining confidentiality of information at all times in compliance with Trust policies, procedures, protocols and guidance.

Induction
KMPT is committed to supporting all new staff and this includes a comprehensive Induction programme. The programme's duration depends on role and can be from 2 days to 4 weeks (not always consecutive days).
The Induction programme will provide an excellent opportunity for new staff to meet and engage with KMPT’s subject specialists and include the required training.
The programme may involve some travel to the various training venues across Kent and Medway, the main ones being Canterbury, Maidstone and Dartford.

Probation Period
