Job responsibilities As a key member of the Cancer Pathways Team, the post holder will monitor patient pathway progress and support the achievement of the national cancer targets. The post holder will work with all members of the Cancer Tumour Site Multidisciplinary Team (MDT) to facilitate the delivery of weekly MDT meetings. Responsibilities include: Facilitate data collection for diagnosed and suspected cancer patients and enter the information onto appropriate database(s). Monitor and facilitate the progress of patients on cancer diagnostic/treatment pathways. Provide information to support achievement of the national cancer waiting times targets. Prepare and present on weekly PTL meetings with the Team Leader and ASM. The post holder will act as the focal point for the multidisciplinary team and will be an essential and integral part in the smooth running, administration, and organisation of a tumour site specific MDT service. This is a unique administration role that requires flexibility, attention to detail, initiative, and excellent communication skills. CANCER WAITING TIMES MONITORING/DATA COLLECTION To establish and maintain a register of all newly diagnosed and suspected cancer patients. Collate information from a variety of sources such as pathology, surgery, chemotherapy, and radiotherapy systems. Update the cancer waiting times database for monitoring 31- and 62-day target on a daily basis. Monitor and facilitate patients progress (tracking) on the cancer pathways, following the Cancer Management Team escalation process to highlight, prevent and resolve delays. Liaise with clinicians, nursing, and other admin staff to determine the reasons for delays with cancer waiting times targets. To facilitate collection of the national cancer dataset (Cancer Waiting Times Dataset) by: Collating information from proformas, case notes, clinics, and MDT meetings. Discussion with clinicians and clinical nurse specialist. Electronic retrieval of prospective and retrospective data from somerset, CERNER and other IT system the trusts. To collate data from referring hospitals required for Cancer Waiting Times. To log actions of meetings and be responsible for ensuring that such actions have been carried out. To work with the MDT members to ensure that appropriate action follows the MDT and that general practitioner are advised of decisions. To provide cross cover for other Cancer Pathway Coordinators during periods of planned leave, sickness, absence or as requested by the Team Leader. Ensure as part of the Cancer Pathways team that all patients on cancer pathways are treated within the national cancer target waiting times.
