Our Short Breaks service based in the Grindon Area of Sunderland are seeking to recruit a Care Co-ordinator (Level 1) who can deliver exceptional standards of person-centred care and support to meet various support needs of individuals, which may include physical disabilities, learning disabilities, autism and/ or mental health needs.
You will use or TLAP (Think Local Act Personal) strategy in promoting independence, co-production, and active participation. Your day to day activities may include:
Helping with preparation of meals & drinks.
Support with domestic household tasks
Administer & record medication
Support with money management
Encouraging community trips and facilitating customers participation
Adhere to health & safety legislation
Cleanliness & intimate personal hygiene
Liaising with families, schools, colleges and other Multi-Disciplinary Agencies to ensure person centred support plan is completed.
