Job summary The Falls Prevention team is dedicated to providing an all-encompassing, responsive, and high-quality shortterm falls prevention service throughout West Kent. Our commitment is to ensure that every referred patient on our caseload undergoes a thorough assessment of how their condition impacts their daily functioning. We craft care pathways rooted in safety and evidence-based practices, ensuring that each patient is attended to by a clinician possessing the requisite skills and competencies. Following best practices and evidence, we systematically assess, plan, implement, and evaluate patient care, actively involving them in decision-making processes regarding their treatment. Main duties of the job You will work closely with the people who use our services, families, friends and carers and play an important contribution in how people experience our services.
