Billericay PCN are seeking an experienced advance nurse practitioner to work as part of a dynamic multi-disciplinary team on a full-time basis, across our 6 practices. Duties will include the following; Provide advanced nursing care, assessment, and treatment to patients of all age groups. Attend home visits to patients who are unable to attend the practice due to mobility issues, chronic illness, or other medical reasons. Collaborate closely with the healthcare team to ensure comprehensive and patient-centred care. Conduct thorough assessments of patients' health conditions, including physical examinations, medical histories, and symptom evaluation. Order and interpret diagnostic tests and investigations. Develop and implement evidence-based treatment plans for patients, including prescribing medications, administering treatments, and providing health education. Provide guidance on managing chronic conditions and promoting a healthy lifestyle. Offer counselling and support for emotional well-being, mental health concerns, and lifestyle modifications. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: Registered nurse (RN) qualification with active registration with the nursing and midwifery council (NMC). Advanced nurse practitioner qualification (ANP) at master's level or equivalent. Additional qualification or certification in independent prescribing (V300) or willingness to obtain this qualification. Experience working within a primary care setting. Demonstrated competence in conducting home visits and managing patients in their own homes. Excellent clinical assessment, diagnostic, and decision-making skills. Strong communication and interpersonal skills to establish rapport with patients, families, and multidisciplinary teams. Ability to work autonomously, make independent judgments, and prioritize workload effectively. Proficiency in using electronic health records (EHR) and other relevant software applications.
