Job summary An opportunity has arisen for a motivated, enthusiastic and innovative individual to join our pulmonary rehabilitation team You will have a keen interest in the field of respiratory care with extensive experience, post nursing qualification. You will provide valuable support to patients with long term respiratory conditions who are referred to the pulmonary rehabilitation team; you will also act as a role model to staff working outside of the expert team You must possess excellent interpersonal skills as well as the drive, vision and initiative to motivate others and meet the demands of the service. You will be experienced at maintaining patient records and databases and have knowledge of the current respiratory pathways. This post is a permanent post and to ensure that the requirements of the service are met, is full time. The post holder will work within both the hospital and community setting. Main duties of the job Main duties of the job: You will be expected to work collaboratively with patients and professional staff to delivering high quality care to patients who are attending the pulmonary rehabilitation programme. You will be expected to triage referrals and undertake patient assessments to then allow for an individual prescription of exercise for those patients attending the programme. In addition to being a qualified nurse, the successful candidate will also be able to demonstrate excellent computer/ IT skills to include Microsoft Office Package, and be enthusiastic about the field of smoking cessation and health promotion Experience in clinical audit and data collection is desirable. You must be able to communicate effectively at all levels, have an understanding of managing your own caseload, demonstrate effective teamwork and participate in ongoing audit programmes.
