About the service Our Blantyre Housing Support Service offers an engaging and dynamic work environment where each day brings unique experiences. We are a dedicated team, committed to delivering exceptional person-centred care to an individual. We support adults with learning and physical disabilities who have complex health and communication needs 24 hours a day in their own homes. There are 5 different locations throughout North and South Lanarkshire Airdrie, Blantyre, Hamilton, Motherwell and Rutherglen. You will receive regular supervision and attend regular team meetings. You will also have peer support from your colleagues in the Blantyre service as well as your colleagues from across the wider service. You will have support from your Supervisor who is based in the Blantyre service, the other 2 Supervisors from the wider service and the Registered Manager. As a new Support Practitioner you will complete all relevant face to face training and eLearning and shadow with experienced colleagues before lone working. Our Team strives to empower those we support to lead fulfilling, independent lives through tailored support and engaging activities. Our core principle is value-based communication, customising our support to align with the unique requirements of each individual. Who do we support? We currently support a female who has lived in the service for a number of years, someone new will be moving in over the next few months and this is a great opportunity to be involved in creating a new service for an individual while supporting the lady who already lives there through this process. We are looking for motivated and confident individuals who will work in a person-centred way to ensure consistent support for the individual supported. The individual we support does not communicate through speech so you will be required to work hard to develop and understand how this individual communicates and respect their need for routine. The individual has complex health needs and you will receive full training in understanding her support. You will liaise with other health professionals and the different support departments within the organisation. There is a full support plan in place, this is regularly reviewed and updated and you will be responsible with the rest of the team for doing this. The individual has their own vehicle that you will be required to drive on her behalf. Personal care, support with finances and administration of medication is part of the role. The individual has her own unique interests which include spending time with her family and friends, rebound sessions, hydrotherapy, storytelling sessions, nights out at discos, shows and meals, bowling, cinema. You will support the person to enjoy her activities of choice in a safe way and to maintain and develop her relationships. What s the shift pattern? Our Blantyre Service provides round-the-clock support every day of the year. This is a lone working role and there are full and part time vacancies within the Blantyre service with a shift pattern combining of early and late shifts and wakened nights on a rota-based system. We currently support a female who has lived in the service for a number of years, someone new will be moving in over the next few months and this is a great opportunity to be involved in creating a new service for an individual while supporting the lady who already lives there through this process.
