We have an exciting opportunity for an Advanced Nurse/Clinical Practitioner to join our super GP practice in Crawley, West Sussex. We aim to ensure that the highest levels of care are delivered to all our patients, particularly in areas such as minor illnesses and acute, on-the-day appointments. This is a highly supportive practice with a long-standing team including GPs, Paramedic Practitioners, Advanced Nurse Practitioner, Practice Nurses, HCAs, and many more Additional Roles.

Main duties of the job
To assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions relating to minor illness.
To undertake home visits, primarily for acute conditions, as assessed by your duty doctor, and within the boundaries of professional qualifications.
The post holder will use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment.
Job responsibilities
Diagnosing and treating patients presenting with minor illness
1. Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team.
2. Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral.
3. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
4. Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary.
5. To undertake home visits, primarily for acute conditions, as assessed by your duty doctor, and within the boundaries of professional qualifications.
6. To communicate at all levels within the team ensuring an effective service is delivered.
7. To maintain accurate, up-to-date healthcare records appropriate to the consultation.
8. Ensure evidence-based care is delivered to the highest standards.
9. Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service.
10. To participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.
11. Contribute positively to the effectiveness and efficiency of the team and work colleagues.
Pathological specimens and investigatory procedures
1. Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc. Perform investigatory procedures requested by the GPs.
Administration and professional responsibilities
1. Participate in the administrative and professional responsibilities of the Practice team.
2. Ensure System 1 is kept up to date, with accurate details recorded and amended.
3. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.
4. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.
5. Attend and participate in Practice meetings as required.
Training and personal development
1. Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the Practice. It is the individual's responsibility to remain up to date with recent developments.
2. Participate in the education and training of students of all disciplines and the induction of all members of the Practice staff where appropriate.
3. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
4. If it is necessary to expand the role to include additional responsibilities, full training will be given.
5. The post-holder will participate in any training programme implemented by the Practice as part of this employment.
6. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

* Qualified Advanced Nurse/Clinical Practitioner.
* Evidence of appropriate knowledge base.
