This is an amazing opportunity for a Registered Nurse to join our expanding multi-disciplinary team at Greyfriars Surgery.
We are looking to recruit a Practice Nurse who will complement the current Practice Nursing and Medical Team to:
* provide high quality care to patients within a primary and community care setting in accordance with CQC regulation, Practice Protocols and Policies.
* have an interest in supporting people living with long term conditions (training will be provided as required)
* develop the required skills, knowledge and experience in other specialist practice nursing areas as required
The successful candidate will be able to work both autonomously and as part of a wider multi-disciplinary primary care team.
The candidate must be able to commute within the Practices geographical area and have access to the appropriate means of transport.
A presentation will be required at interview; the topic will be advised when invited for interview.

Main duties of the job
* Assess, diagnose, plan, implement and evaluate interventions/treatments for patients.
* Diagnose and manage both acute and chronic conditions and ensure the delivery of best practice for those registered patients, whilst developing and supporting General Practice Nursing duties.
* Work with patients in order to support compliance with and adherence to prescribed treatments.
* Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
* Promote and deliver evidence-based care for patients presenting with aural conditions.
* Childhood and Adult immunisations and vaccinations including Influenza and COVID vaccines.
* National Screening Procedures and campaigns.
* Work collaboratively with the Practice Nurse team and wider PCN team.

Job responsibilities
Job Purpose & Summary: The post holder is a qualified Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the Surgery. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and supporting the training of GP Registrars and other nursing staff as required.
Key responsibilities:
* Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
* Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
* Diagnose and manage both acute and chronic conditions, integrating both drug-and non-drug-based treatment methods into a management plan.
* Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. (Dependent on Practitioner)
* Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
* Carrying out consultations via various methods including telephone triage, face-to-face, video consultations and in some cases; home visits.
Person Specification
Experience
* Experience in General Practice is preferred but not essential.
* Experience working with patients with Long Term Conditions.
Qualifications
* Current NMC registration.
* Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
* Compassionate manner with an ability to demonstrate kindness in all aspects of your work.
* Management of patients with long term conditions.
