We Have an Exciting Opportunity
We are looking for a Punjabi Speaking Senior Complex Needs Navigator to join our vibrant community team. This role is about making a tangible difference by working closely with street-based services and healthcare professionals to engage with people facing physical and/or mental health issues, substance use, and rough sleeping.
Where: Ealing
Full Time Hours: 37.5 per week
Full Time Salary Range: £32,002.35 - £34,214.21, Pro rata for part time hours
Allowance: £4,133.14 Inner London Weighting
Contract Type: 12 months fixed term until 31st March 2026
Responsibilities
About the Role
* Engage and Empower: Coordinate and lead a solution-focused approach, helping service users move away from street-based living and into appropriate services.
* Street Outreach: Identify and establish contact with rough sleepers and beggars, providing rapid assistance through street outreach at various times and locations.
* Personalised Support: Manage a small caseload of complex clients, developing personalised action plans to discourage rough sleeping.
* Multi-Agency Collaboration: Participate in multi-agency initiatives and meetings to reduce street activity and ensure holistic care.
* Healthcare Access: Work proactively to ensure service users have ongoing access to healthcare and substance misuse treatments.
* Data and Analysis: Contribute to the statistical analysis of rough sleepers and participate in street counts as needed.
About You
* Language Skills: Fluent in Punjabi.
* Relevant Experience: Experience in substance misuse, alcohol misuse, mental health, or related fields.
* Complex Needs Expertise: Proven experience working with people who have multiple and complex needs.
* Multi-Agency Collaboration: Skilled in working within multi-agency settings and facilitating meetings.
* Excellent Communicator: Strong communication, negotiation, and advocacy skills.
* Relationship Builder: Ability to build and maintain relationships with a wide range of services.
* Team Player and Autonomous Worker: Comfortable working both independently and as part of a team.
* IT Proficiency: Good levels of IT literacy, including MS Office applications.
* Reflective Practitioner: Ability to critically evaluate interventions, service delivery, and identify areas for development.
