PROGRAM PURPOSE:
The National Association of Chronic Disease Directors (NACDD) is a national public health non-profit agency focused on improving the health of the public by strengthening state and national leadership and expertise for chronic disease prevention and control. NACDD promotes health and reduces the burden of chronic disease through various programs and initiatives in collaboration with the CDC’s Division of Nutrition, Physical Activity, and Obesity and the Division of Population Health.
The Public Health AmeriCorps (PHA) program supports the recruitment, training, and development of the next generation of public health leaders. Service Members will be placed across twenty states to advance efforts around food and nutrition security, safe physical activity access, social connectedness, and health equity.
SPECIFIC SERVICE FUNCTIONS AND RESPONSIBILITIES:
The Service Member will support YMCA of Metro Atlanta programs related to advancing health equity and improving health outcomes through improved food and nutrition security, safe physical activity access, and social connectedness. Responsibilities include:
* Implementation of specific health equity outcomes.
* Collaboration with partner organizations.
* Support of the Blood Pressure Self-Monitoring program and member enrollment.
* Scheduling and implementing health/screening opportunities at YMCA branches and early learning centers.
DESCRIPTION OF ANTICIPATED SERVICE SCHEDULE:
* 100% In-person service hours
* Service hours are to be completed Monday-Sunday between 5:00am and 8:00pm
* A maximum of 20% of service hours may be training, education, or similar approved activities
* A maximum of 10% of service hours may be fundraising activities
DESIRED SKILLS & COMPETENCIES:
* Ability to understand, interpret data, and draw conclusions.
* Excellent interpersonal skills and a desire to serve the community with empathy.
* Proficiency in Microsoft products.

REQUIRED COMMITMENT:
* Complete 1200 service hours within one year (02/03/2025 - 02/02/2026).
ORIENTATION AND TRAINING:
* Participation in pre-service orientation and PHA-specific training.
* Program-specific training and any other necessary trainings.
EVALUATION AND REPORTING:
* Complete quarterly assessments, monthly reflection forms, and participate in performance evaluations.
* Submit service timesheets biweekly.
PUBLIC HEALTH AMERICORPS ELIGIBILITY AND REQUIREMENTS:
* Must be at least 18 years old.
* High school diploma or equivalent.
* Must be a citizen, national, or lawful permanent resident alien of the U.S.
* Satisfy National Service Criminal History Check eligibility criteria.
* Live within commutable distance of the Host Site.
OTHER PROGRAM ELIGIBILITY AND REQUIREMENTS:
* Willingness to comply with Host Site vaccination requirements.
* Completion of Youth & Teen Safety course and other required trainings.
PUBLIC HEALTH AMERICORPS PROGRAM BENEFITS:
