Clinical Pharmacy To take referrals and triage them from a wide range of health and social care professionals to develop pharmaceutical support packages for patients with complex medication needs in order to ensure they are competent in the use of their medicines. Undertake patient facing clinics independently within the scope of your prescribing competency Undertake medication reviews of PCN patients ensuring involvement of the residents, relatives, care home staff and GP practices and liaising with the GP, Community Pharmacist, and Care Home Manager regarding the outcomes. Proactively lead initiatives that promote a good quality service. These include: Intervention monitoring and reporting for prescribing alerts Medication error reporting. Clinical audit and research projects. Reporting adverse drug reactions to the Committee on Safety of Medicines via the yellow card reporting scheme. Promote adherence to National Patient Safety Agency advice. Investigation of complaints and incidents relating to pharmacy team Monitoring, inspection and destruction related to controlled drugs within primary care. Provide specialist clinical knowledge and expert advice on complex prescribing related issues where requested. Deliver elements of clinical governance standards related to prescribing and medicines management contained within Department of Health guidance. Encourage safe repeat prescribing in the practices by facilitation development of protocols, audit and feedback. To provide specialised medicines and therapeutic advice to inform GP practices on priorities and actions, and the treatment of patients Management Provide Monday to Friday, 8:30am 4:30pm on-site clinical seniority presence and support within our PCN Pharmacy hub To line manage, mentor, lead and supervise other pharmacists and technicians employed by WIN PCN To manage performance, competency assessment and professional development through clinical audit, appraisal process and one to one meetings. To be a role model for the pharmacy team and actively demonstrate the organisational and departmental values and behaviours. To challenge behaviour that is not in line with WIN PCN values and help make improvements, through coaching and mentoring. To have direct staff managerial responsibility if required, including managing performance and professional development. Provide timely, clear and concise written evidence of HR related interactions to the PCN Strategy Manager Leadership To be the clinical lead pharmacist for Warrington Innovation Network To provide daily operational leadership to manage and prioritise workload amongst pharmacy team members Laise with the PCN Operations Lead / PCN Strategy Manager around any safety concerns regarding workload Liaise with the PCN GP Clinical Safety Lead around any safety concerns regarding workload To lead on the development of a positive safety culture within the pharmacy team Work with the senior leadership team and clinical safety lead to implement the PCN Safety Strategy To lead on the use of an evidence-based approach to influence prescribing and ensure patient-centred and cost-effective choice of therapy. To lead on clinical audit and projects for the team to support safe medicines use and prescribing for the PCN. Research and Audit Lead and report on clinical audits and improvement projects identified within WIN PCN and member practices Support the safe use of antimicrobials and compliance with policies and procedures relating to medicines and risk management processes. Evaluate interventions and action plans implemented, for effectiveness, re-audit and research. Critically analyse research findings and their implications for practice. Training and Education To provide an ethos of continuous learning and development within the PCN pharmacy team Provide an annual training schedule for the pharmacy team for protected learning time events To deliver clinical education and training sessions for the pharmacy team Support/arrange for other pharmacy team members to deliver educational sessions within the team Attend member practices to deliver education and training on medicines issues and support/arrange for other members to deliver educational sessions To support the development of the pharmacy team through clinical supervision, education and training as required. Observe telephone and face to face sessions conducted by the pharmacy team to check competencies and support learning and development. Maintain an up-to-date competency matrix for all members of the pharmacy team. To be involved in the supervision and training of healthcare professionals where there is an identified need for providing support for medicines Provide timely, clear and concise written evidence of any learning/training needs and supportive action taken To take responsibility for own personal development and continuing education and training as necessary to ensure a high level of competency required in carrying out tasks. To remain aware of national developments in pharmaceutical care and undertake Continuing Professional Development in accordance with the requirements of the General Pharmaceutical Council. Location During working hours, the individual will be required to be based on site at our WIN PCN Pharmacy Hub or from other work locations within the Warrington area as required to meet service demands/needs of the business. Given the geographical nature of the PCN, you will be required to travel between member practice premises and multiple community locations as part of your role.
