Come and be a part of a team that makes a real difference every day Main duties of the job o To work within a multi-disciplinary team, in the delivery of high-quality patient care to patients & relatives. o Support shift leader ensuring that all documentation is up to date and report any changes in the patients' condition. o To contribute to the development and evaluation of nursing practice. o To demonstrate effective communication skills both written and verbal between patients and members of the multi-disciplinary team. o To communicate effectively with patients and their relatives. o To advise on the promotion of health & prevention of illness. o To liaise with appropriate clinical specialists in relation to individual patient needs e.g. Doctors, Occupational therapists, physiotherapists and In Reach matrons.
