We are a key provider of Primary Care services in the Norfolk & Waveney system working with the local GP practices. NPC has been sub-contracted to provide a home visiting service to the Practices in the Norwich PCNs. This comprises of a team of clinicians working across the Norwich PCNs. The staff provides additional Primary Care support to that currently being provided by individual practices. Supported by a Service and Clinical Lead, the HVS team are responsible for working with all HVS participating Practices focusing on proactive support to patients residing at home (including care homes). The aim of the HVS is to reduce the number of avoidable admissions into hospital and to deliver more consistent, enhanced services from Primary Care.
