Main duties of the job
We would like to meet someone who has completed educational preparation to work in general practice, and who has knowledge and skills in all aspects of general practice nursing.
We are looking for someone with skills and competence to provide:
* Treatment room care such as for wound assessment and management, ECGs, spirometry, anticoagulant monitoring, etc.
* Support for management of long-term conditions, including diabetes and respiratory conditions.
* Vaccinations and immunisations for all ages, including the childhood vaccination programme.
* In particular, we are looking for someone with an interest and qualifications in sexual health and cervical screening.

We have a patient population of approximately 9,000 patients and pride ourselves on providing excellent care and support for registered patients. You will work alongside a supportive team who are constantly striving to create the best environment, not only for their patients but also their team members. The post holder will work collaboratively with the general practice team to meet the needs of patients and support the delivery of independent contractor general practice services.
Job responsibilities
Job summary
The post holder is responsible for the delivery of general practice nursing services to the registered practice population. The focus of the role is on the provision of evidence-based care in respect of disease prevention and early detection, disease monitoring and management, health promotion, supporting acute health needs and management plans, and providing proactive support and shared management for patients with long-term conditions. As an autonomous and competent practitioner, the post holder is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management and monitoring of patients' health needs. In addition, they will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nursing leadership where required.

Qualifications
* Registered first level nurse.
* Current NMC registration.
* Community nursing specialist qualification (or equivalent).
* Working towards or willingness to work towards Masters level study.
* Independent and/or supplementary nurse prescribing qualification.
* Smear takers certificate/qualification.
* Pertinent postgraduate diplomas in long-term conditions management, such as Diabetes, Respiratory, Family Planning, Wound Care, Minor Illness (or a willingness to undertake postgraduate studies according to the needs of the practice).
* Clinical supervision training and experience.
Experience
* 3 years post-registration experience.
* Recent primary care or community nursing experience.
* Experience of nurse-led management of long-term/chronic conditions.
* Experience of implementing protocols and clinical guidelines.
* Experience of quality improvement and audit.
* Experience of all aspects of general practice nursing.
* Awareness of current issues in nursing and general practice.
* Experience of facilitating learning in practice.
* Experience of formal/informal teaching and of mentoring pre-registration nursing or midwifery students.
* Experience of supporting team members and coordinating team activities.
Knowledge
