An exciting new role became available for a Social Worker with older people mental health experience.
What will your responsibilities be?
You will be responsible for managing a caseload of older adults with diverse care and support needs. This role involves collaborating with multi-disciplinary colleagues and other agencies to develop personalized care and support plans. You will support service users and carers in creating plans that utilize community resources to achieve positive outcomes. Additionally, you must have the ability to travel across a wide geographical area to complete work for the Older People’s Mental Health team.

Qualifications and Experiences :
You must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree level or equivalent qualification in Social Work are also required.
