We are currently seeking a part-time Registered Mental Health Nurse to become part of our dedicated Community Mental Health Team. Based at Aylesbury at the Whiteleaf Centre, our team is committed to providing exceptional care and treatment to adults with mental health challenges. Our goal is to deliver patient-centered care of the highest quality, while supporting our staff in their roles and fostering their professional growth.
As part of the Aylesbury CMHT, you will work alongside a multidisciplinary team that includes medical professionals, psychological therapists, social workers, occupational therapists, RMNs, and support workers. We also collaborate with third-sector workers who specialise in employment, housing, and social needs. To bridge the gap between patients and clinicians, we have also introduced Peer Support Workers who bring the invaluable perspective of lived experience.
Responsibilities:
1. Perform specialist nursing assessments in collaboration with service users and their families/carers, and present findings to the team.
2. Work in partnership with service users and their families/carers to create and oversee treatment plans that ensure safety, promote well-being, and support independence and community inclusion.
3. Deliver evidence-based interventions following NICE guidelines to promote recovery and prevent relapse in various settings, and regularly review care and outcomes.
4. Provide professional consultation to multi-agency services, meetings, and forums, and contribute to the duty provision within the team.
5. Participate in continuous personal development, clinical education, and support for students and non-registered staff, and contribute to the planning, development, and evaluation of nursing services.
