Overview:
The role of the Care-Coordinator provides a vital service and is a crucial member of the team. The role is fast paced as well as incredibly rewarding. Listed below are some of the key responsibilities – although not exhaustive it was created to give a good snapshot of the day to day actions that a Care coordinator undertakes.
Responsibilities:
1. Adhering to Core Values, policies and procedures.
2. Applying diversity and equal opportunities policies in the workplace.
3. Interviewing prospective candidates for the role of carer.
4. Ensuring all client details, care plans and risk assessments are in place.
5. Planning to cover holidays, training, and planned absence.
6. Taking part in the On Call out of hours’ service on a rotational basis.
7. Organising care worker rotas to consider travel time and staff skill mix.
8. Answering the telephone promptly and professionally, dealing with any enquiries or queries.
9. Ensuring the provision of high-quality care services to vulnerable people living in their own home.
10. Undertaking staff supervisions and appraisals.
11. Liaising with other professionals (e.g., GP, Commissioners, district nurses).
12. Working alongside the manager to ensure regular reviews of service users are completed in a timely manner.
13. Ensuring all electronic rostering systems are up-to-date.
14. Submitting all travel expense forms within the set time frame.
Requirements:
1. Strong skills in planning, scheduling, prioritising, and time management.
2. Ability to establish and maintain effective professional working relationships.
3. Desire to make a real difference and improve an already well-established business.
4. Minimum of 1 year’s experience in a similar role.
5. Relevant qualification, such as Business Studies or Social Care – Desirable.
6. Driving licence and access to own vehicle.
7. Extremely well organized, with excellent planning and prioritising ability and high attention to detail.
8. Experience with scheduling/HR/recording systems – Desirable.
9. Ability to work unsupervised.
10. Strong administration skills and ability to manage multiple workloads.
11. Self-motivated and flexible, with a willingness to participate in an on-call system for out of office hours.
12. Previous Domiciliary Care experience – Desirable.
13. Strong problem-solving skills.
14. Knowledge of office software (Word, Excel).
