Tarian Group is a GP Partnership operating across two practices in Cardiff. This post is a full time position working 4 days per week across both sites (Greenmount Surgery & Danescourt Surgery). We are a dynamic and friendly practice with a truly collaborative approach to MDT working and the successful candidate will work alongside GP, nursing and pharmacy colleagues.
The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing care for patients via face to face, telephone consultations and home visits.
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The post holder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.

Main duties of the job
The main responsibilities of the role include:
1. Undertaking a variety of duties including surgery consultations, telephone consultations and queries.
2. Dealing with queries, paperwork, reports and correspondence in a timely fashion.
3. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
4. Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
5. Screening patients for disease risk factors and early signs of illness.
6. Developing care plans for health in consultation with patients and in line with current practice disease management protocols.

We operate across two further GP Practice sites within Cwm Taf Morgannwg and professional collaboration and support is actively encouraged across all practices.

Qualifications
* Registered Practitioner (NMC/HPC)
* Knowledge of Safeguarding Adults/Children including Mental Health Act and Mental Capacity Act and Deprivation of Liberty Safeguards
* Knowledge and understanding of current policy and issues in professional practice, compliance, standards and the wider NHS
* Registered as a non-medical independent prescriber
Experience
* Significant previous experience of working within specialty area
* Demonstrable experience of complex decision making
* Experience of multi-professional working
* Relevant experience where a degree of autonomous working has been required
