Job summary Are you a qualified nurse looking to provide skilled clinical care to patients as part of the Virtual Ward team across a range of community settings? An exciting opportunity has arisen to join the Virtual Ward Service and be part of a friendly, supportive multi-disciplinary team. This position is part of a 7 day service based at St Marys Hospital, Isle of Wight. The Virtual Ward programme is designed to facilitate early and supported discharge of patients from acute medical beds and to prevent hospital admissions for patients with unstable conditions in the community. Our team consists of clinical staff supported by senior medical professionals, working across the Island. We closely align with colleagues in other evolving community services, notably the Technology Enabled Care team and the Urgent Community Response team. As a valuable team member, you will be responsible for the clinical assessment of patients who may need admission to the ward. This will be conducted in various community settings and patients' homes, so a full driving licence and car ownership are essential. Experience: Experience managing long-term condition patients within community and acute settings Experience with managing conditions such as respiratory issues, frailty, heart failure, or other long-term conditions (LTCs) is highly desirable as we work towards a wider portfolio of conditions managed on the ward. Main duties of the job The post holder is expected to work in partnership with key stakeholders to provide skilled clinical care to patients as part of the virtual Ward team in a full range of community settings. In times of extreme pressure, you may be asked to support the Urgent Community Response team within the Division, in line with your experience, competence/ skill set.
