This is an exciting and unique opportunity to join the newly developed Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team in South London and Maudsley (SLaM).
The FIND Team will provide safe transition for inpatients discharged from the inpatient Forensic Services to the community, providing specialist assessment, treatment and consultation to ensure a smooth, safe and clinically justified pathway for individuals with mental illness, offending history and learning disability with or without neurodevelopmental disorder.
The three forensic services of SLaM, South West London and St Georges (SWLSTG), and Oxleas together comprise the South London Mental Health and Community Partnership (SLP). The SLP's overarching goals centre on bringing care closer to home, improving patient pathways and outcomes by delivering excellent care in better environments. The FIND provision is one such development, and the SLaM FIND Team will work closely with the FIND Teams in each of the other Trusts to expedite service-users' transition through inpatient settings to community provision.
Main duties of the job
1. Work in partnership with MDT colleagues to support a case load of service users with complex needs, using evidence-based and service user-centred principles.
2. Provide specialist Occupational Therapy for a caseload of service users, completing assessments, interventions and outcome measures underpinned by Model of Human Occupational and the Recovery Approach.
3. Participate in the planning, development and evaluation of services provided.
4. Provide placement education opportunities and supervision for OT students on practice placements and junior members of staff as required.
5. Carry out multidisciplinary activities where appropriate, e.g., involvement in team quality improvement projects.
6. Work collaboratively and effectively within a multi-disciplinary community team liaising with inpatient services and community-based services as required.
7. Support service users to identify recovery goals, develop daily living skills and improve motivation and confidence to live independently.
8. Support service users with any physical health or physical disability needs while interfacing with local services to ensure adaptive equipment is sought and fitted as necessary.

Job responsibilities
1. Select and apply OT assessments underpinned by MOHO Theory for a designated caseload to support case formulation and intervention planning.
2. Work with service users to identify OT goals as part of the overall care plan, working within the CPA process.
3. Plan and implement individual and/or group interventions, in collaboration with the service user, using graded activity to achieve therapeutic goals.
4. Identify any significant risk factors in line with local policies, with support from senior staff.
5. Monitor, evaluate and modify treatment for service users with multiple needs to measure progress and ensure effectiveness of intervention.
6. Consider both physical and mental health needs as part of the occupational therapy process.
7. Assess the occupational and vocational needs of the service user group, signposting and/or supporting access to community and employment opportunities where appropriate.
8. Provide clinical training and supervision to the service users' extended support network as appropriate.
9. Provide information, advice, and assessments to the team regarding the suitability of placement.
Person Specification
Training, Qualifications Registration
* Registered Occupational Therapist
* Member of OT professional body and/or special interest group.
* Post-graduate training.
Experience
* Post registration experience as an OT working in mental health.
* Clinical experience of working with adults who have a learning disability or autistic adults.
* Experience working in groups and extensive clinical experience including individual and group work.
* Experience supervising junior staff and/or students.
* Knowledge and/or experience of working as a Care Co-ordinator.
* Experience placing, reviewing and working with people who live in a residential, supported accommodation or nursing care setting.
* Experience working in an inner-city, multi-cultural environment.
* Experience working with people with challenging behaviours.
Knowledge / Skills
* Understanding of Mental Health disorders and treatment models.
* Knowledge of legislation relevant to Mental Health.
* Knowledge of recovery model and other theoretical models in mental health rehabilitation and continuing care.
* Knowledge of the principles of CPA (Care Programme Approach).
* Knowledge of the principles of risk management and risk assessment.
* Training and facilitation skills.

Salary: £42,939 to £50,697 per annum inclusive of inner HCAs.
