Job Title: Adult Senior Practitioner – Community Independence and Wellbeing Team
Location: London Borough of Hammersmith and Fulham
Rate: £35 per hour
An exciting opportunity has recently become available for an Adult Senior Practitioner. They are looking for a new member to join the Community Independence and Wellbeing Team in the London Borough of Hammersmith and Fulham.
What will your responsibilities be?
You will conduct comprehensive assessments of adults in the community to determine their needs and develop personalized care plans. You will provide ongoing support and intervention to ensure the well-being and safety of vulnerable adults. Additionally, you will collaborate with multidisciplinary teams and external agencies to coordinate services and resources for clients.

Qualifications and Experiences:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
