Job overview
To provide a person-centred service to users in intermediate care and their carers in order to facilitate timely discharges from the service, thereby increasing bed availability.
To work in partnership with the social work team and other members of the Multi Disciplinary Team (MDT) to meet service users’ individual needs and provide support for carers in accordance with departmental policies and good professional standards.
Main duties of the job
* Support the Intermediate Care Social Work team with all practical matters to facilitate an expedient discharge from the Intermediate Care Unit in Salford.
* Assist in identifying suitable residential/nursing home placements, offering advice and guidance to individuals and their families during this process.
* Gather information on care home vacancies across the city, and maintain excellent working relationships with the management of these establishments.
* Develop extensive knowledge of local resources within Salford which can offer assistance to the individuals we work with.
* Work across the MDT with other professionals to support a timely discharge from the Bevan Unit.
* Provide emotional support to individuals and their families.
* Record information in a timely and thorough manner using appropriate social care and health records.

Detailed job description and main responsibilities
* Provide advice and practical assistance, as identified by a social care professional, to service users and their carers to facilitate timely discharges back to their own home.
* Assist service users and their carers to identify and move to an appropriate residential/nursing care setting when required (e.g., escorting).
* Offer emotional support and reassurance where appropriate to service users and family/carers.
* Promote and encourage service user choice and ensure that service users’ needs and wishes are understood by service providers.
* Gather and maintain accurate, timely information regarding care home availability and knowledge of local resources to facilitate timely discharges from intermediate care bedded units.
* Liaise with members of the social work team, other professionals, and carers to meet service user needs and proactively address any issues that are preventing timely discharges.
* Understand the roles and responsibilities of other disciplines within integrated services.
* Report to line manager on progress of discharges, residential/nursing care bed availability, and any areas of concern or challenges.
* Keep electronic social care records up to date in accordance with departmental standards.
* Maintain daily reports on all service users requiring transfer into residential/nursing care to support discharge target requirements and evidence where delays are occurring.
* Undertake any necessary training.
Person specification
Experience in social care
* At least 2 years of experience in adult social care.
* Experience working in an Intermediate care setting.
* Experience of facilitating discharges from a hospital setting.
