Main duties of the job The Role As an Advanced Clinical Pharmacist, you will be a cornerstone of healthcare delivery and will play a critical role in the multidisciplinary team, covering the Acute Virtual ward that is in operation in our locality. Here is what you will do Leadership within the Virtual ward regarding medicines and medicines governance. Medicine reconciliation. Structured medication reviews including complex polypharmacy and deprescribing. Prescribing and accurate record keeping regarding medications. Answering medication queries raised by the multidisciplinary team and providing clinical support to the Acute Virtual Ward Team. Discharge facilitation. Work within integrated care across several multi-disciplinary teams including, Mental Health, District Nursing Discharge teams, Acute GPs, and Social Workers.
